Pendaftaran Online Dr.Sardjito 4.0.2

This application is designed for patients who wish to register online at Dr. Sardjito Central Public Hospital.

Online registration is currently only available for patients who already have an MRN at Dr. Sardijito Central Public Hospital for outpatient treatment.

Online registration can be done for outpatient clinic appointments within H+1 to H+14 days from the registration date by entering: MRN, Date of Birth, Choice of appointment day, and doctor for regular clinics. The appointed doctor is the Attending Physician (Dokter Penanggung Jawab Pelayanan).

Registration for the day before (H-1) must be completed before 2:00 PM, otherwise, the patient will be registered on the following day.

A patient can only register once with one doctor on the same day.

Patients who have registered online will receive an SMS reminder of their appointment schedule.

Doctor's schedules are subject to change at any time.

After completing online registration, you will receive a registration proof that can be printed and brought on the day of appointment.

If you wish to reprint your Online Registration Proof after registering, you can do so by Checking the Registration Proof Code on this page's form.

The examination queue number is based on the order in which patients arrive on the appointment day.

For Emergency cases, please proceed to the Emergency Room at Dr. Sardjito Central Public Hospital.

Online Registration Proofs should be presented at the Online Registration Counter (Outpatient Building 1st Floor ICC Building 2nd Floor).

Patients who have registered online are expected to arrive on time (at least 1 hour before the Doctor's Clinic schedule).

Patients who do not show up and do not re-register at least 1 hour before their doctor's schedule time will have their registration automatically removed from the system, and their spot may be taken by another patient.
